If you have previously filed a report with the Columbia Police Department, either online or with an officer, and your original report type is listed below, you may file a supplemental report using our online system. To do this, you will need the case number given to you, when you filed your original report. If your original report type is not listed below or you do not have your case number, you will need to either contact (803) 252-2911 and have an officer respond to you, or contact (803) 545-3500 and speak with our Telephone Response Unit.

When completing the supplemental report, you do not need to go into the details of what happened, in the Incident Description section, unless there is addition incident information that was not included in the original report. If you are only adding information, such as serial numbers for products, you can put something similar to “This supplemental report provides information not available at the time of the original incident report.” in the Incident Description section.
